To provide an on-call social worker to speak with families about their concerns and to connect them with appropriate referrals, services, education and support; and thus reducing the physician’s time spent counseling family caregivers about eldercare issues.

Empowering Latinos to Use Health Care More Effectively
To select and train leaders from within the Latino community as Lay Health Advisors (LHA), who will provide two functions: to teach fellow immigrant Latinos how to seek and utilize appropriate health care, and to serve as ongoing health referral sources within their communities.
