Professional Services The staff is available to assist donors and their advisors to develop charitable gift plans that meet their philanthropic goals and interests to promote good health in Columbus. Credit Card Gifts We accept credit card gifts over the phone and online using your Visa, Mastercard, American Express or Discover Card. Mail/E-mail - This option is under construction. There are several options for mailing a gift to the Columbus Medical Association Foundation for the benefit of the Physicians Free Clinic. You can print our gift form, fill out the information for your gift and mail to us. You can also complete the information to make a gift and e-mail the form.
